Whiteside County Community Health Clinic is a FTCA Deemed Facility: This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

Mission:

To provide primary and preventative medical, dental and behavioral health care across the region and to receive individuals from all life cycles, with dignity and respect in a caring environment.

Sliding Fee:

No one will be refused care based on inability to pay. To ensure that income or lack of insurance is not a barrier to patients who are under 200% of the federal poverty level, each patient’s family size and income will determine their rate based on a sliding fee scale. The minimum fee per visit is $25.00. For those with Medicaid, Medicare, or private insurance, the copay amount for your plan will be due at the time of service.

Services:

MAT Program: Medication Assisted Treatment for opiate addiction. If you or a loved one is suffering from opiate addiction, our program may be for you. Please call (815) 626-2230 Ext. 8 for more information and get scheduled today.

Group Sessions:

  1. E.S.H. (Experience, Strength and Hope) groups are focused on recovery and life skills. These sessions take place here at our clinic on Tuesdays from 1:30 p.m. to 3:00 p.m. and Wednesdays fron 1:00 p.m. to 3:00 p.m. Call us at (815) 626-2230 Ext.8 and speak with a behavioral health receptionist to be apart of this group.
  2. Grandparents group is a support system for grandparents that are currently raising their grandchildren. The group meets every 3rd Tuesday of the month. Please call (815) 626-2230 ext. 1209 if you would like more information.
  3. Mens group is a "men only" group that meets on a weekly basis to deal with issues related to being men in today's society including developing relationships and pursuing health and happiness. For more information and to be a part of this group, please call (815) 626-2230 ext. 8.
