Tuesday, Wednesday, Thursday - 7:30am to 6pm

Behavioral Health:
Monday, Wednesday, Thursday, Friday - 8:00am to 4:30pm
Tuesday - 8:00am to 6:00pm

Dental:
Monday, Wednesday, Thursday - 7:30am to 4:30pm
Tuesday - 7:30am - 5:30pm
Friday - 7:00am to 4:30pm

Sliding Fee:
No one will be refused care based on inability to pay. To ensure that income or lack of insurance is not a barrier to patients who are under 200% of the federal poverty level, each patient’s family size and income will determine their rate based on a sliding fee scale. The minimum fee per visit is $30.00 for a medical or behavioral health visit, and $35 for a dental visit. For those with Medicaid, Medicare, or private insurance, the copay amount for your plan will be due at the time of service.  You may apply for a discount at the Front Desk. After one year, you must reapply.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.  
  • You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.

Services:

  • Medical: Provides primary and preventative health care integrated with behavioral health and is staffed by physicians, nurse practitioners, physician assistants and licensed clinical social workers.
  • Dental: Provides preventative and comprehensive dental treatment for children age 20 and below including dental school exams. Basic dentistry for adults only - no dentures, bridges, or braces.
  • Behavioral Health: Behavioral Health: Provides therapies and support services to individuals experiencing emotional distress, mental health symptoms, and substance use disorder.
  • Family Planning: Provides confidential services to all women of childbearing age, including pap exam, birth control supplies, pregnancy testing and emergency contraception. Testing and treatment of STD’s are available to all men and women over age 12.
  • Equality Health Clinic: Provides safe, confidential, and comprehensive services including medical, dental, behavioral health, HIV and sexually transmitted infections, and healthy lifestyle consultation.
